This agreement is entered into between _______________, hereinafter referred to as the “client,” and Peak Five Ventures, LLC, hereinafter referred to as “Peak Five.”

Purpose of Relationship:
This coaching relationship aims to enhance the client’s lifestyle through a comprehensive personal coaching approach, encompassing various methods such as videos, worksheets, assignments, one-on-one discussions via phone, video chat, texts, and emails.

Non-Medical Nature:
This relationship is not a substitute for medical advice or treatment. Neither Peak Five nor any individual coach is providing a diagnosis for any medical condition, whether mental or physical.

Responsibility for Medical Relationships:
While your coach may hold a doctor’s degree, they are not acting as your medical doctor or your healthcare provider. The client is responsible for maintaining relationships with their own medical doctor(s) and/or healthcare provider(s).

Medication Guidance:
Coaches may discuss medications, mental health, and physical health to understand the client’s lifestyle, but they will not prescribe any medication, whether pharmaceutical, natural, or otherwise. Clients should consult their medical doctor before implementing any recommendations.

Plant Medicines:
Discussion of plant medicines is optional, and their use is not a requirement of this program.

Opinions and Recommendations:
All discussions and recommendations from Peak Five are based on personal opinions and experiences. Clients should thoroughly vet and consider these before incorporating them into their lifestyle.

Legal Compliance:
Peak Five does not endorse the violation of any laws related to the use or possession of plant medicine. Clients are responsible for researching and understanding applicable laws in their area.

Results Disclaimer:
The client understands that results are neither guaranteed nor implied.

Indemnification:
The client agrees to hold harmless and indemnify Peak Five, including any agents acting on behalf of Peak Five.
